If you are a new patient, you will need to bring the following to your first appointment:

1.) A government issued picture identification card. Examples would include:

  • Driver’s license 
  • Non drivers license ID
  • Government work ID

If you do not have any of the above forms of identification or are unsure if yours meets the requirements, contact our office during business hours for assistance.

2.) Current valid insurance card. Insurances accepted are: 

  • Amerihealth
  • Carefirst Blue Cross Blue Shield 
  • DC Medicaid 
  • Maryland Medicaid
  • Medicare
  • Health Services for Children with Special Needs (HSCSN)
  • Trusted Healthcare 
  • United Healthcare
  • Commercial carriers that offer out of network benefits 

3.) Co-payments 

Please check with your insurance company prior to your appointment to determine the amount of your co-pay. Co-payments are due at the time of services.

As a patient, it is in your best interest to familiarize yourself with all aspects of your insurance plan. All services may not be covered in your benefits package and/or you may be responsible for a co-payment.  Additionally, you should be certain that your physician is listed as a participating provider. 

In the event that your insurance is not valid at the time of services rendered, you will be solely responsible for the full amount of your office visit, labs, and/or any procedures, which will be due at the time of services.   
 

Self Pay

We offer self pay options to individuals without insurance coverage. Please be aware that payment in full is due at the time services are rendered. Please contact our Center for rates. Prices are subject to change.

We also offer financial assistance through a Sliding Fee Scale Program. For further details, please read the information provided under Sliding Fee Scale or contact our office during business hours. 


Payments

Payments to your account can be made in person, over the phone, or by logging into your patient portal account.  Payment methods accepted are Visa, MasterCard, Discover Card, American Express, and cash.

Sliding Fee Scale

Elaine Ellis Center of Health offers services at reduced fees to patients who are uninsured or underinsured and who are eligible for a discount based on family size and family income. For more information please contact our office during business hours.
